If you have health insurance, you will need to contact your carrier at the number on the back of your card to request authorization to see us out-of-network. If out-of-network, you will be responsible for the full fee (see below) and self-submitting statements to your insurance for reimbursement.
PLEASE NOTE: Some Blue Cross Blue Shield plans will NOT routinely cover test administration for ADHD evaluations. Further, Kaiser does not routinely authorize the full duration of testing. In such cases, additional testing fees (not covered by insurance) may apply.
Unfortunately, we cannot accept Medicaid or Medicare for either a secondary or primary policy.

STANDARD NOTICE
“Right to Receive a Good Faith Estimate of Expected Charges”
Under the No Surprises Act
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

ESTIMATES
ADHD Assessment Procedures and Fees
Total cost for the evaluation is $1,350.00
(The total fee can be split into two payments of $675.00, with the final payment due before the feedback session).
*If you are insured with an in-network carrier (e.g., BC/BS or Kaiser) that does not fully cover test administration or deems this to not be a medically necessary service, a $500.00 estimated fee is assessed.
Our ADHD evaluation process is comprehensive and designed to provide a clear understanding of your unique strengths and needs. We do not just screen for ADHD, but also assess for a range of other possible concerns to ensure an accurate diagnosis and helpful recommendations.
Initial Intake (1-2 sessions):
These sessions may be conducted individually or with a significant other or parent. We gather background information, discuss your current concerns, and review any previous evaluations or records you provide.
Test Administration (1-2 sessions, 4-6 hours total):
You will complete a variety of standardized tests and questionnaires. These are designed to assess attention, memory, executive functioning, intellectual abilities, adjustment and coping skills, and screen for learning issues and other mental health symptoms.
Follow-Up Interviewing and Records Review:
We may conduct additional interviews and review relevant records to clarify your history and current functioning.
Feedback Session:
Once the assessment is complete, we will meet to discuss the results. You will receive a summary of the feedback session that reviews our findings and recommendations. Additional charges may apply if requesting a full evaluation report.
Autism Assessment Procedures and Fees
Total cost for the evaluation is $1,800.00.
(The total fee can be split into two payments of $900.00, with the final payment due before the feedback session).
Our autism evaluation process is comprehensive and individualized, designed to provide a clear understanding of your strengths, needs, and any areas of concern. We assess for autism as well as other possible conditions to ensure an accurate and thorough diagnosis. Our goal is to provide you with increased understanding and guidance for next steps.

These sessions may be conducted individually or with a significant other or parent. We gather detailed background information, discuss your current concerns, and review any previous evaluations or records you provide.
Test Administration (1-3 sessions, 6-8 hours total):
You will participate in a variety of standardized tests and activities. These are designed to assess social communication, behavior, cognitive and adaptive functioning, as well as screen for co-occurring mental health or learning concerns.
Informant Interviews and Data Collection:
We may gather additional information by interviewing parents, caregivers, or teachers, and by collecting school records or seeking informant ratings. This helps us understand your functioning across different settings and from multiple perspectives.
Follow-Up Interviewing and Records Review:
Additional interviews or record reviews may be conducted to clarify your history and current functioning.
Feedback Session:
After the assessment is complete, we will meet to discuss the results. You will receive a summary report that reviews our findings and recommendations.
Comprehensive Neurodevelopmental Disorder Evaluation
Total cost for the evaluation is $2500.00.
(The total fee can be split into two payments of $1250.00 with the final payment due before the feedback session).
A comprehensive neurodevelopmental disorder assessment may be requested to explore concerns related to ADHD, autism, and learning differences such as dyslexia or dyscalculia. This evaluation is more in-depth and provides a thorough understanding of learning and other neurobehavioral functions.
Initial Intake (1-2 sessions):
These sessions may be conducted individually or with a significant other or parent. We gather detailed background information, discuss your current concerns, and review any previous evaluations or records you provide.
Test Administration (2-4 sessions, 8-12 hours total):
You will complete a wide range of standardized tests and activities. This includes assessment of attention, learning and memory, executive functioning, reading, mathematics, and writing skills, as well as personality or temperament. We also screen for co-occurring mental health or neurodevelopmental concerns.
Informant Interviews and Data Collection:
We may gather additional information by interviewing parents, caregivers, or teachers, and by collecting school records or seeking informant ratings. This helps us understand your functioning across different settings and from multiple perspectives.
Follow-Up Interviewing and Records Review:
Additional interviews or record reviews may be conducted to clarify your history and current functioning.
Feedback Session:
After the assessment is complete, we will meet to discuss the results. You will receive a comprehensive summary report that reviews our findings and recommendations.
