INFORMANT REPORTS (Adult ADHD Evaluations)

 

Your clinician may request input from people who observe you in different settings. Obtaining additional information from people familiar with you can help inform our evaluation and guide recommendations.  Below are some questionnaires that are helpful parts of your evaluation.

Once complete, you can upload forms to your secure patient portal. If your informant prefers to provide forms to us directly, they can email us at Info@TheBrightsideCenter.com (provided you are comfortable with them emailing form). Forms can also be dropped off at or mailed to our office at 203 E. 3rd St., Moscow, ID at anytime using the mail slot (for drop-off, please place in a sealed envelope addressed to our office). You are also welcome to fax forms to 833-411-1240.

• CURRENT SYMPTOMS: Other Report Form - This form is completed by a parent, roommate, significant other, or close friend who is currently familiar with you. 

• CHILDHOOD SYMPTOMS: Other Report Form - This form is completed by a parent/caregiver familiar with your childhood experience.

• CHILDHOOD SCHOOL PERFORMANCE: Other Report Form - Completed by a former teacher or another individual who had the opportunity to directly observe you in the school environment. This form is optional, we realize that many adults do not have contact with a former teacher/tutor.
